Online Registration
Whether you are a new or current patient,
using our online pre-registration
capability, you can complete your
demographic and insurance information
and even input your personal health
history prior to your visit.

To register as a user on our
secure web site please Click
Here > Once you have created
a User ID and Password you can access any
of our secure features. We ask that you
create a unique, secure password that only
you know. It should contain only letters
and numbers.
To keep your account secure, please do not
to use the names of your spouse, children,
favorite sports team or other easy to guess
passwords. During the registration process
you will be asked for an email address.
This must be a real address that you have
access to. You will verify your email in
step two of the signup process.
Please make sure to remember
your user ID and Password for later sessions.
For security purposes the office cannot
give out User IDs over the phone. If you
have forgotten your password and remember
your user ID you can click on the "forgot
my password" button located on the
log in screen.
Prescription Renewal
You may securely request your prescription
renewal online or if you prefer you may
use our prescription refill phone line at
574-826-2203. Either way your prescription
will be faxed to the pharmacy. If you choose
to use the online option our staff will
communicate back with you through the secure

Appointment Request
You may securely request an appointment
directly through our interactive web site,
without having to call to the office. Click

After Hours
Your calls to our main office number will
be answered by our answering service. You
will be prompted to leave a detailed message
including your name and phone number. The
answering service will then relay the information
to the physician who will respond as quickly
as possible. Evening and Weekend calls are
for Emergency conditions only.
Routine problems are handled more effectively
during business hours when your medical
records are easily accessible.
